A case manager serves as a vital advocate and coordinator for clients in need, offering personalized support and guidance. They assess the client’s needs, develop and implement individualized care plans, connect clients with essential resources and services, and continually monitor progress to ensure the client’s well-being and goals are met. Through their expertise and compassionate approach, case managers empower clients to navigate complex systems and overcome challenges, ultimately improving their overall quality of life.

A care coordinator acts as a liaison between the client and the professionals, navigating the coordination of care for the client. The care coordinator will identify appropriate providers, facilities, and treatment modalities, as well as schedule appointments and diagnostic studies, on the client’s behalf. Additionally, care coordinators educate clients regarding their care options and can assist with any required documentation. It is the coordinator’s priority to ensure that the client receives all necessary treatments and services in a cohesive and timely manner, so that the client can simply focus on getting the care they need without all of the stress of navigating complex systems.

Cognitive assessments are performed using a clinician-directed program that gives people a clear picture of their brain fitness. It includes a series of simple computer application tests and associated reports for clinicians to monitor your brain fitness as well as data-driven recommendations for brain training to help address your specific needs. It helps measure cognitive functions that may impact brain wellness such as memory, executive function, attention, visual spatial, verbal function, problem solving and working memory.

A traditional healing system that restores the balance and flow of energy throughout the body, mind, and soul. Energy healing works directly with the physical, emotional, and spiritual aspects of well-being. While energy healing can be a terrific complement to the treatment of many medical conditions, it is especially useful in supporting recovery related to mental health disturbances. Reiki is a form of energy healing. The practice is based on the concept that we all have unseen “life force energy” flowing through our body. A Reiki practitioner gently moves his/her hands just above or on your clothed body, with the intention of reducing stress and promoting healing by encouraging a healthy flow of energy. Reiki promotes relaxation and may help with anxiety, depression, and pain.

Family coaching is a process where one or more members of a family partners with a PBWI coach to address goals that will ultimately improve their family dynamics. Family coaching is not therapy. A coach helps one create action plans to achieve desired goals and results. Family dynamics are the interactions between family members, how they relate to one another and how the family functions. There are all sorts of tools that a coach may use to help families communicate about their shared values, beliefs and goals to see where they are aligned and where they may be out of sync.

Fitness training is a wellness modality that balances five elements of good health. A certified fitness trainer will develop and implement a program that may include aerobic fitness, strength training, core exercises, balance training, and flexibility and stretching. The personal trainer will work with one on one to help design an engaging exercise regimen, help with form, ensure the client is working all muscle groups safely and more. Being physically active can improve brain health, help manage weight, reduce the risk of disease, strengthen bones and muscles, and improve overall health and wellbeing. Functional training is a classification of exercises which involves training the body to perform activities in daily life. At any point during your wellness journey, our board-certified internal medicine physican is available to provide medical guidance to the client and the wellness team. This may involve review of diagnostic studies, collaboration with your primary care or subspecialty physicians, or direct consultation to evaluate any underlying medical issues that the client may be experiencing. Additionally, one can feel confident that all of our wellness providers have been personally vetted by our medical director, with the goal of offering high quality, evidence-based, interdisciplinary wellness services.

Medical nutrition consultation is provided by a registered dietitian who will evaluate the nutritional needs considering one’s health conditions. This is done by taking a thorough history and understanding of the current dietary intake, accessing physical measures such as BMI and waist circumference, and reviewing lab results to provide an optimal dietary plan based on the individual’s health needs. Optimizing nutrition provides the necessary elements for the body and brain to function at its highest capacity.

Our board-certified internal medicine physician will ensure that patients’ medications are used safely and effectively. Through comprehensive medication reviews, the physician assesses all prescriptions, over-the-counter drugs, and supplements to optimize dosages and prevent harmful interactions. Patients receive clear guidance on how to take their medications, along with support to improve adherence and address any side effects. We will coordinate with other healthcare providers as needed and offer ongoing monitoring to adjust treatment plans for the best possible outcomes. Mindfulness-based stress reduction (MBSR) is a standardized therapeutic approach to mindfulness meditation. Mindfulness meditation is a mental training practice that teaches to slow down racing thoughts, let go of negativity, and calm both mind and body. It combines meditation with the practice of mindfulness, which can be defined as a mental state that involves being fully focused on “the now” so one can acknowledge thoughts, feelings, and sensations without judgment. One will begin to learn about how pain is processed in the nervous system and how to rewire the brain to change one’s experience of pain. Pharmacogenetic testing analyzes clinically important genetic variations in your DNA to help determine how your genes may affect medication outcomes. Results can inform your doctor about how you may break down or respond to certain medications commonly prescribed to treat depression, anxiety, ADHD, and other psychiatric conditions.

Physical Therapy is used to preserve, enhance, or restore movement and physical function impaired or threatened by disease, injury, or disability.
